Family Care will conduct Sports Physicals on Friday, May 3rd and Friday, May 10th. Information will be coming home this week. If you are interested, you must complete the Sport Physical Form prior to the appointment.

Currently enrolled and incoming kindergarten Putnam County Schools students are eligible to participate in programs offered for their grade level.
