The Family Care Bus will be available to complete physicals for students the week of May 8th. If your student needs a physical completed, they must sign up in the office and have the physical parental consent form filled out to be seen.
The physical parental consent form will be near the sign in sheet for students to grab. Be sure that your student gets one for you to fill out and sign (front and back of form) for them to be seen on the Family Care Bus.
